DNP Program Completion Form - This form is submitted to the Office of Graduate Studies when planning to graduate.The DNP Program Completion Form must be submitted to the Office of Graduate Studies by midterm of previous term prior to graduation. (See "Graduation" category in the WSU Graduate Catalog.)
DNP Program Plan Form - This form is submitted to the Office of Graduate Studies to determine students plan of study. Before the student has completed 16 semester credits with a grade point average of at least 3.0, the student, in consultation with the advisor, must submit a completed DNP Consortium Program Plan Form to the Office of Graduate Studies. (See "Application for Admission to Candidacy" category in the WSU Graduate Catalog.)
