ATEP Application Forms

The following forms are to be used to fulfill the requirements for application to the ATEP.
 

Admitted Student Forms

The following forms will be completed during fall camp each year and maintained in the office of the ATEP Program Director.  Completion of ALL forms is required prior to the start of the students assigned clinical experience
 

HERS 292 – Principles in Athletic Training

The following forms are required to be completed and turned in during the first week of class in the fall semester.  Clinical education rotations assigned to a preceptor WILL NOT be allowed until all paperwork is complete.

Did You Know??

The independent Board of Certification, Inc (BOC) nationally certifies athletic trainers.

Athletic trainers must pass an examination and hold an entry-level bachelor's or master's degree to become an ATC.  50 hours of continuing education is required over a two year period to retain certification. (www.bocatc.org)
