We are transitioning to a new online accommodation request system for the spring 2013 semester.
To start the process, if you are already registered with Access Services, please go to the application to create your profile in our system upon which everything will be linked. You will need to do this step only one time.
Access Services staff will enter your eligible accommodations and notify you when you can submit your accommodation requests.
Please complete the online accommodation request form for each class in which you would like to receive accommodations. Your professors will be notified of your eligibility on the first day of class for anything submitted before the start of the semester. After the semester starts, your professors will be notified within 2 business days of your request.
Please schedule a meeting with them to discuss how your accommodations will be provided this semester. You might find these suggestions helpful in starting the discussion with your professor.
If your schedule changes after you've submitted your requests, please enter a new accommodation request for any added classes, and put a note in the comments section at the bottom of the form that the class replaces one which was dropped.
