Athletic Training Room Policies and Procedures Coat Room A coat and book bag room located across from the men’s locker rooms in the downstairs hallway of Memorial Hall is provided for the use of the Athletic Training Staff. All book bags, coats, rainwear, boots, etc. should be stored in the coat room during hours of operation. The coatroom must be locked at all times. The Athletic Training Room key can be obtained from an ATC/R and will open this door. Stereo The 1997-98 Athletic Training Club donated a receiver, CD player and speakers for use in the Athletic Training Room. These are very expensive items and need to be treated with the utmost care. The volume of the stereo should not impair the ability to verbally communicate at normal conversational levels. Music selection should represent a broad range of choices to suit every ones tastes. The supervising ATC/R’s reserve the right to censor music choices. Computers The computers are provided for athletic training and athletic insurance purposes. Use of a computer is prioritized for injury tracking, athletic insurance and generating documents for the athletic training program. Student personal use is a privilege. ATS's may use the computer for e-mail, word processing and Internet research during non-peak athletic training times. All personal documents should be saved to a floppy disk and not on the hard drive. All documents not relating to the functions of the Athletic Training Program will be deleted. At all times University policies will be adhered to regarding use of e-mail and the internet. Library A small reference library composed of texts, flash cards, CD-ROMs, etc. is provided for staff and student use in the athletic training room. This reference material is used daily and needs to remain available. Therefore, these materials need to stay in the athletic training room. Materials may be checked out overnight but must be returned the next day. Completing the sign out sheet in the library cupboard may check out an item. Remember these materials are provided for everyone so treat them with care. Keys At times it is necessary for students to gain access to the athletic training room when an ATC is not available. For this purpose two extra sets of keys are available for the student to check out. It is the student’s responsibility to notify their supervising ATC if they need a set of keys. Contacting an ATC and having the ATC sign out a set to the student may check out keys. The student’s name, date checked out, date returned and the supervising ATC/R’s initials should be recorded. The sign-out sheet and keys are located in the locked drug cabinet. Keys should be returned as soon as the event or need for the keys is completed. Use of University Vehicles ATS's will be expected to drive university vehicles to and from outdoor practices and events. Please follow the guidelines and rules attached to the clipboard in the vehicle. If you have any questions or problems with the vehicle contact your supervising ATC/R immediately! The vehicle assigned to the athletic training area must be put away each night in the proper location. ATS's will need to fill out a student employment form to be covered under the University’s insurance. ATS’s may be asked to take an athlete to the emergency room or to a physician’s appointment. University vehicles should be used for this. Vehicles should be cleaned and all supplies and equipment should be removed daily! ATS's are expected to drive responsibly and follow all laws. OTC Medications Over the counter medications are to be kept secured in the locked drug cabinet. A Team Physician or ATC/R may only administer drugs. When administering medications an instruction card regarding the dosage and warnings should be given to the individual. The following information should be recorded on the Drug Administering Sheet: name, date, medication, amount, dosage and ATC initials. Athletic Training Kits should not contain medications. Small dosage packs may be checked out for travel. A record of administering needs is to be completed at that time. All items that are removed from the storage room should be signed out on the inventory check out sheet. The type, quantity, dates and name of individual should be noted. When receiving supplies the same information should be noted on the inventory check-in sheet. At the end of the month the ATS should enter the information of total supplies checked out and in into the database. When it appears that a supply is running low the supervising ATC should be notified. The key to the storage room is located in the paperwork cabinet. Modalities Any electrical modality treatments performed in the Athletic Training Room must have a prescription from an ATC/R and be supervised by an ATC/R. A prescription and treatment form must be filled out prior to any treatment administered. The prescription should then be placed into the tx log book categorized by sport. Every tx should be recorded on this form with the care giver’s initials. Only students who are properly trained may administer tx’s. You must have your supervising ATC/R evaluate your performance on each modality before giving tx’s. Only students who have successfully completed HHP 393 Therapeutic Modalities may suggest tx prescriptions and must have an ATC/R sign it before giving the initial tx. This form should then be placed in the current injury sport file. Once the injury has been successfully treated the form should be filed in the athletes individual file. If the athlete was referred to a physician then a referral form should be completed by the supervising ATC/R and this must accompany the athlete to the appointment. If the athlete is seeing a CMH Sports Medicine Physician then a patient questionnaire and an insurance information form needs to be completed. All corresponding paperwork to the injury should be attached to the original injury report form and filed chronologically in the athlete’s file. If an athlete is receiving rehabilitation in the Athletic Training Room then a rehabilitation form should be filled out. It should include in proper progression the tx, exercise, stretching etc. with the number of set, reps, and days. The athlete should check mark each day the program is completed. When the rehabilitation is complete this form should be filed with the original injury report form in the athletes file. Every athlete must complete a green personal data sheet, an emergency information blue card, a physical, and a preseason screening. These forms should be filled in the athlete’s individual file. Every athlete entering the Athletic Training Room for treatment must sign in on the daily tx log. It is very important that this form be displayed and available for athletes to sign in. It is a very necessary part of record keeping for the Athletic Training Program. These forms should be totaled monthly and placed in the daily tx log file.
