ENROLLMENT PROCEDURES
A child must have the following forms on file before beginning care at the Center and participate in a program orientation session with the director.

  • Enrollment form with signed emergency, fieldtrip and media releases
  • Health summary signed by physician
  • Immunization record
  • Emergency contact card
  • Food Program sheet
  • Personal Data form

WAITING LIST


Children may enroll as full-time or part-time students. Requests for enrollment are processed on an age-eligibility and date-of-application basis with the following priorities:

  1. Families already affiliated with the center

  2. Families from the University community (students, faculty, staff, alumni)

  3. Families from the wider community
