The right to humane care and treatment. Patients will be treated with
respect, consideration and dignity. Patients can expect that their
personal convictions and beliefs will be taken into account when seeking
help and that the convictions and beliefs of the health service personnel
will not adversely affect the right to rational and appropriate care. |

The right to accurate information, to the extent known, concerning
diagnosis, treatment and prognosis of an illness or health related
condition. This includes appropriate alternatives to care at the health
service when requested. |

The right to a second opinion regarding diagnosis or treatment.
The right to participate in decisions, which are made regarding your
healthcare and treatment. |

The right to know who is counseling, caring for or treating you. The
health service staff members name and title should be visible or stated on
introduction. |

The right to written
information regarding the scope and availability of services at the health
service. |

The right to information regarding fee for services and how to pay for
charges incurred. |

The right to confidentially of medical records |

The right to be informed of any research aspect of your care and the right
to refuse to participate. Such refusal will not jeopardize your access to
medical care and treatment. |

The right to be seen by a fully credentialed provider. Further
information on the credentialing and recredentialing process is available
upon request from the Health Service. |

Patients will
provide full information about their illness or problem to allow
complete evaluation and treatment |

Patients will ask
sufficient questions to ensure that they understand the cause of their
illness or problem as well as the health providers’ recommendation for
continuing care. If the course of treatment is not acceptable for any
reason, the matter should be discussed the director of the health
service, the medical director or the vice president for student
affairs. |

Patients have the
responsibility to show courtesy and respect to health service staff and
other patients |

Patients are
responsible for correct use of their personal ID and not to lend it to
others |

Patients are
responsible for canceling or rescheduling appointments so that time may
be given to another student. |

Patients are
responsible for not giving their medications to others to use. |

Patients are
responsible for communicating with the health providers if their
condition worsens or does not follow the expected course. |

Patients are
responsible to used identified methods of expressing grievances and
suggestions whenever indicated rather than remaining silent in order to
assist the health service in improving the quality of health care and
education services.
