Winona State University Immunization Requirement
All students are required to complete a WSU Student Immunization Record. Those students born on or before December 31, 1956, are required to complete the form, but are not required to supply immunization dates. All others (except those with a physician signed medical or notarized conscientious exemption) are required to have the following immunizations:
- Measles, Mumps and Rubella (MMR): One dose is required after your first birthday.
- Diphtheria/tetanus (tD): Must be within the last ten years.
International student and those students returning to the U.S. from countries where TB is prevelent are strongly encouraged to have a tuberculin (TB) skin test administered upon registration.

If you already have an immunization form on file at Rochester Community & Technical College, you can get a copy of your record from RCTC Health Services and bring to WSU Rochester Center Student Services and we can send it down via intercampus mail. (Immunization records are only kept for the last two school years at RCTC.)
