Patients will be treated with respect, consideration and dignity.<\/li>\n<li>Patients can expect that their personal convictions and beliefs will be taken into account when seeking help and that the convictions and beliefs of the health service personnel will not adversely affect the right to rational and appropriate care.<\/li>\n<li>The right to accurate information, to the extent known, concerning diagnosis, treatment and prognosis of an illness or health related condition. This includes appropriate alternatives to care at the health service when requested.<\/li>\n<li>The right to a second opinion regarding diagnosis or treatment. The right to participate in decisions, which are made regarding your healthcare and treatment.<\/li>\n<li>The right to know who is counseling, caring for or treating you. The health service staff members name and title should be visible or stated on introduction.<\/li>\n<li>The right to written information regarding the scope and availability of services at the health service<\/li>\n<li>The right to information regarding fee or services and how to pay for charges incurred<\/li>\n<li>The right to confidentiality of medical records<\/li>\n<li>The right to be informed of any research aspect of your care and the right to refuse to participate. Such refusal will not jeopardize your access to medical care and treatment.<\/li>\n<li>The right to be seen by a fully credentialed provider. If the course of treatment is not acceptable for any reason, the matter should be discussed with the director of the health service, the medical director or the vice president for student affairs.<\/li>\n<li>Patients have the responsibility to show courtesy and respect to health service staff and other patients<\/li>\n<li>Patients are responsible for correct use of their personal ID and not to lend it to others<\/li>\n<li>Patients are responsible for canceling or rescheduling appointments so that time may be given to another student<\/li>\n<li>Patients are responsible for not giving their medications to others to use<\/li>\n<li>Patients are responsible for communicating with the health providers if their condition worsens or does not follow the expected course<\/li>\n<li>Patients are responsible to use identified methods of expressing grievances and suggestions whenever indicated rather than remaining silent in order to assist the health service in
