complete a&nbsp;<a href=\"https:\/\/warriorspace.winona.edu\/ProcessDirector\/form.aspx?formid=e4ddddcc-cb8d-4dec-a73f-970da6148c86&amp;pid=cbc76cfa-e332-489a-a868-237dcf390893\" data-type=\"URL\" data-id=\"https:\/\/warriorspace.winona.edu\/ProcessDirector\/form.aspx?formid=e4ddddcc-cb8d-4dec-a73f-970da6148c86&amp;pid=cbc76cfa-e332-489a-a868-237dcf390893\" target=\"_blank\" rel=\"noreferrer noopener\">WSU Student Immunization Record<\/a>.&nbsp;<\/p>\n\n\n\n<p>Unless you have a physician-signed medical or notarized conscientious exemption, you are required to have the following immunizations:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Measles, Mumps, and Rubella (MMR): 1 dose is required after your first birthday<\/li>\n\n\n\n<li>Diphtheria-Tetanus (TD): Must be within the last 10 years&nbsp;<\/li>\n<\/ul>\n\n\n\n<p>If you were born on or before Dec. 31, 1956, you must complete the form but you don\u2019t need to supply immunization dates.&nbsp;<\/p>\n\n\n\n    <div class=\"panel panel--white\">\n        <div class=\"panel__container\">\n            <div class=\"panel__intro\">\n                            <div class=\"h3\">Recommended Vaccinations<\/div>\n                        <\/div>\n            \n\n<p>The&nbsp;American College Health Association (ACHA)&nbsp;also recommends the <a href=\"https:\/\/www.acha.org\/ACHA\/Resources\/Guidelines\/ACHA\/Resources\/Guidelines.aspx#immunization-recommendations\" data-type=\"URL\" data-id=\"https:\/\/www.acha.org\/ACHA\/Resources\/Guidelines\/ACHA\/Resources\/Guidelines.aspx#immunization-recommendations\" target=\"_blank\" rel=\"noreferrer noopener\">following immunizations:<\/a><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hepatitis B\u2014series of 3 vaccines<\/li>\n\n\n\n<li>2nd dose of Measles, Mumps, and Rubella (MMR)&nbsp;<\/li>\n\n\n\n<li>Meningococcal Vaccine &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;<\/li>\n<\/ul>\n\n\n\n<div class=\"wsu-block tabs\">\n\n    <div class=\"tabs-nav\">\n        <button class=\"tabs__toggle\" tabindex=\"0\" aria-expanded=\"false\" id=\"tabbed-1\">Hepatitis &amp; Meningococcal<\/button>\n<button class=\"tabs__toggle\" tabindex=\"0\" aria-expanded=\"false\" id=\"tabbed-2\">COVID<\/button>\n<button class=\"tabs__toggle\" tabindex=\"0\" aria-expanded=\"false\" id=\"tabbed-3\">Travel-Related<\/button>\n    <\/div>\n\n    <div class=\"tabs-content\">\n        <button class=\"tabs__mobile__toggle\">Hepatitis &amp; Meningococcal<\/button>\n<div class=\"tabs__content__item\" aria-hidden=\"true\"><div><p>WSU is required to inform all first-time students about:<\/p>\n<ul>\n<li>the transmission, treatment, and prevention of hepatitis A, B, and C<\/li>\n<li>the risk of meningococcal disease<\/li>\n<li>the availability of meningococcal aria-hidden=\"true\"><div><p>COVID-19 vaccines are strongly recommended for all students.<\/p>\n<p>Health Services has vaccines and booster shots available for free.<\/p>\n<p>Please bring your insurance information, student ID and your vaccine card (if receiving second or third dose) to your appointment.<\/p>\n<p>After receiving your COVID-19 vaccine, please fill out the <a href=\"https:\/\/winona.az1.qualtrics.com\/jfe\/form\/SV_3HObFC7pk6O4ojX?Q_SDID=SD_2mp8nvChpiKNCPH&amp;Q_JFE=qdg\" target=\"_blank\" rel=\"noopener\">self-report a country where Tuberculosis (TB) is prevalent, you are strongly encouraged to have a Mantoux (TB) skin test administered upon registration.<\/p>\n<\/div><\/div>\n    <\/div>\n\n<\/div>\n\n        <\/div>\n    <\/div>\n\n\n\n\n<h2 class=\"wp-block-heading\">Find Your Vaccination Record<\/h2>\n\n\n\n<p>Your vaccination record (sometimes called your immunization record) provides a history of all the vaccines you received as a child and adult.&nbsp;<\/p>\n\n\n\n<p>This record may be required for certain jobs, travel abroad, or school registration.&nbsp;<\/p>\n\n\n\n<p>Many <a href=\"https:\/\/www.cdc.gov\/public-health-gateway\/php\/communications-resources\/health-department-directories.html\" data-type=\"link\" data-id=\"https:\/\/www.cdc.gov\/public-health-gateway\/php\/communications-resources\/health-department-directories.html\" target=\"_blank\" rel=\"noreferrer noopener\">states have immunization information systems<\/a> (IIS) or databases that you can access online.<\/p>\n\n\n\n<p>You can request a copy of immunization records from Health Services or your primary care
